YOUR OPERATIONAL CHECK LIST- is to be followed by you the Patient as an operational guide to the highest levels of clinical efficacy.

UNDERSTANDING THE USE OF YOUR UNIVERSAL Bi-phasic™ (HT™) IMPULSATOR®

FIRST FIND A COMFORTABLE BREATHING POSITION

 
Prepare to relax in a comfortable armchair, with the HT® IMPULSATOR® on a firm table within the reach of a wall power outlet.
The power cord is contained within the left facing compartment of the travel pack. To prepare for a treatment, insert the plug into a mating voltage wall electrical source.

Patient’s Phasitron® Duo™ Breathing Head is stored CLEAN and DRY in a disposal plastic bag within the right end compartment.
Your Breathing Head interfacing tubing and medications are stored in the front compartment of your travel pack.

Drop your medication directly into the separated Phasitron® Duo Nebulizer bowl. DON’T SPILL MEDICATIONS.
If you are using a 2.25% Racemic Epinephrine dilute with water. Otherwise dilute with normal saline.

GENERAL NEBULIZER SOLUTIONS ARE:

  • Place .5 cc of an Alpha Beta Racemic Epinephrine in the Nebulizer Bowl then dilute with 15 cc of clean water.
  • 20 cc of NORMAL SALINE alone.
  • Your Prescribed Beta bronchodilator (such as Albuterol) diluted with 15 CC of .85% normal saline (supplied by your pharmacist).

There also are other Special Intravenous type Prescription solutions, approved for intra-bronchial topical delivery.


After filling Nebulizer with selected solutions, twist lock The Nebulizer Bowl into the Phasitron® Duo top.

Insert the white and yellow open-end bayonet fittings of the Interfacing Harness into the white PHASITRON and yellow NEBULIZER (color matching) Service Sockets in the top operations panel of the HT™ IMPULSATOR® Respirator.


Twist-lock the three yellow, white and green color-coded bayonet fittings of the interfacing tubing (matching color to color), into your combination Phasitron® Duo - Nebulizer Breathing Head assembly.

Rotate the Bi-phasic PERCUSSION control knob Arrow full (counterclockwise) to EASY, finally, select NEBULIZATION ONLY.

RECRUIT YOUR BRONCHIAL AIRWAYSAND ALVEOLI AS WELL AS, YOUR BRONCHIAL CIRCULATION

Alternatively, after placing the HT Impulsator® travel pack securely on your bed, lie down with your head and shoulders propped up

With Bi-phasic™ Percussion Arrow rotated toward EASY and NEBULIZER ONLY mode selected, push the POWER switch to ON.
Initially, put your lips around the Phasitron® Duo™ mouthpiece and breathe the dense aerosol emission. Continue breathing the dense Aerosol mist through the mouthpiece for about two (2) minutes.

 
You may be a little surprised when you first experience the pulsed air deliveries, during your initial Percussive Breathing.
After about two minutes of Aerosol breathing, move the THERAPY SELECTION switch to Bi-phasic™ IPV®.  Seal lips firmly around the Phasitron® Duo™ mouthpiece and allow the continuous bursts of air to deeply penetrate into your lungs.
During low phase therapeutic lung Percussion, breathe IN and OUT through the mouthpiece as you would NORMALLY breathe, for about five (5) minutes. Feel your lungs being therapeutically Percussed from within. If you don’t feel like breathing, RELAX and let the Bi-phasic™ IMPULSATOR® breathe for you.

The above wave form shows what happens when you physically obstruct and release the
Bi-phasic™ vent hole with your Thumb about every fifteen seconds, causing the Percussive Bi-phasic™ amplitude to be alternately increased for bronchiolar and alveolar therapeutic lung recruitment (TLC™).

 

After about five minutes of low amplitude Percussion, switch from low amplitude to alternate high-low phase Percussion by covering the PERCUSSION AMPLITUDE vent hole with your thumb for alternating periods of about 15 seconds.

Each time you hold your Thumb over the Bi-phasic™ vent hole you will notice that you are increasing your lung Percussion amplitude. This will recruit (increase your lung expansion) as long as you keep the hole covered with your Thumb.
 

You should schedule about ten (10) minutes of intermittent Bi-phasic lung Percussion. This will also help you raise and expectorate (cough up) retained endobronchial secretions.

Note: Your Bi-phasic™ lung mobilization with manually controlled Percussive amplitude, serves to increase your alveolar oxygen delivery as well as “wash out’ your excess carbon dioxide.

During and after treatments when you cough up secretions have tissues available to cough secretions into.

After about 10 minutes of Bi-phasic™ lung recruitment select   NEBULIZER ONLY. Take deep breathes inhaling a dense aerosol deep into your lungs for about 2 minutes for bronchiolar airway saturation.
 
Your scheduled Bi-phasic™ IPV® treatment period should be from fifteen (15) to twenty (20) minutes from start to finish. DO NOT NIBBLE, TAKE A FULL TREATMENT EACH TIME.

Clean up your Breathing Head assembly.

Finally, push the Power switch to OFF with Therapy Selection set to Nebulization only, and rotate the Bi-phasic™ Percussion Arrow counter-clockwise to EASY.

After treatment disconnect the interfacing tubing from the Breathing Head Assembly, disassemble, cleanse and dry as per instructions, later in this document.

The preceding Bi-phasic™ HT™ IMPULSATOR® instructions provide a general profile for mobilizing and recruiting the pulmonary bronchioles and their alveoli with Bi-phasic™ Intrapulmonary Percussive Ventilation (IPV®). Experienced COPD patients may modify their protocols to a more intensive schedule.


As you become increasingly comfortable with your daily IPV® treatments you will gradually increase the amplitude of your lung recruitment impaction. During the last five minutes of your ten (10) minute Bi-phasic™ recruitment schedule, you will progressively rotate the Bi-phasic™ CONTROL KNOB (clockwise) toward AVERAGE. Usually you will feel comfortable within about three days from the time you start the gradual increase in percussive amplitude.

Over time you will discover the advantage of rotating the PERCUSSION control knob (clockwise) of the AVERAGE position to raise secretions, should you experience a transient acute pulmonary infection.

Peripheral lung recruitment is maintained by taking at least two daily IPV® treatments morning and evening. This is in addition to your programmed COPD personal routines.

NOTE: You cannot take too many treatments.

If you have a cold or other more acute lung infections you may need treatments as often as every hour and/or as necessary to keep your lungs open.
